Male to Female Gender Reassignment Surgery
Male to Female Gender Reassignment Surgery (also know as Sex Reassignment Surgery, or SRS) is a complex and irreversible sex change operation and therefore prospective patients must demonstrate a firm commitment to the decision to proceed.
The World Professional Association for Transgender Health (formerly the Harry Benjamin Association) recommends that six objectives must be met before a sex change operation is undertaken.
The patient must demonstrate a desire for sexual reassignment for at least 2 years prior to surgery
A clinical behavioral scientist trained to deal specifically with transexualism must make the diagnosis of Gender Identity Disorder.
The patient must live and work exclusively in his or her chosen gender for not less than 12 months.
The patient must be under psychological or psychiatric care for not less than 6 months prior to surgery.
The patient must have commenced hormonal sex reassignment treatment for not less than 6 months prior to surgery.
Throughout the evaluation process, peer reviews must be evaluated and the patient discussed by the appropriate clinicians.
In order to preserve the integrity and ethics of Transgender Reassignment Surgery, the Medical Council of Thailand now insists that all prospective patients are approved by a Thai psychiatrist. To facilitate this process, Doctor Sanguan requires, one month before intended surgery, a letter of referral from a psychiatrist, a physician, an endocrinologist or a psychotherapist. This needs to demonstrate the person’s suitability for GRS and will help ensure that the examination by the local psychiatrist (which will take place on the day prior to scheduled surgery) does not cause a delay to the surgery schedule.
If the patient can meet these criteria, they will be scheduled for operation. Doctor Sanguan performs GRS in two stages, separated by seven days, a unique and proven process designed to improve appearance and sensitivity. For this reason, and to aid recovery, patients can expect to spend 3-4 weeks in Phuket.

Facial Feminization Surgery
Facial Feminization Surgery (FFS) covers a wide range of surgical procedures designed to create a feminine face on male-oriented features. Occasionally sought by women wishing to reduce masculine facial characteristics, it is more commonly desired by the transgendered community. It is an important factor for those undergoing male to female transition, as the facial appearance invariably provides the first visual clue to a person’s sexual identity.
FFS involves alterations to the eye-lids and brows, nose, jaw, and neck, and will include plastic and reconstructive surgery.

Tummy Tuck (Abdominoplasty)
Abdominoplasty is a cosmetic surgery procedure designed to make the abdomen firmer. It involves the removal of excess skin and fat in order to tighten the muscles of the abdominal wall. It is a treatment often sought by women following pregnancy or individuals who have undergone significant weight loss.
Prior to surgery Doctor Sanguan requires photographs of the stomach area to evaluate and to provide advice, together with a complete medical history (age, height, weight, drug allergies heart conditions etc.). Patients must refrain from using Aspirin and stop smoking for at least 10 days before the operation.
Patients will be hospitalized for 3-4 days, with the removal of stitches some 5-6 days after the operation.
The operation involves the removal of excess skin from the lower abdominal area and the repositioning of the navel to a more natural position. The muscle and sheath below the abdominal skin will also be tightened. Scarring around the navel and on the lower abdomen (below the bikini line) will result from the procedure.
Face Lift (Rhytidectomy)
Rhytidectomy is, typically, cosmetic surgery designed to give a more youthful appearance. As people age, the skin begins to lose elasticity and wrinkles begin to appear. The skin can also start to sag, especially below the chin, in the upper neck and at the skin folds near the corner of the mouth.
As with other plastic and cosmetic procedures, the increase in popularity of face lifts has resulted from great advancements in technology and techniques, and PPS is at the forefront of these developments.
Prior to surgery Doctor Sanguan requires facial photographs, full frontal and profile, to evaluate and to provide advice, together with a complete medical history (age, height, weight, drug allergies heart conditions etc.). Patients must refrain from using Aspirin and stop smoking for at least 10 days before the operation.
Face lift surgery is carried out under general anesthetic, and requires hospitalization for 3-4 days, with stitches removed 5-7 days after the operation.
During the operation excess skin from the forehead, temple, cheeks, chin and neck is excised, and whilst bruising and some swelling is inevitable, this will disappear in 2-3 weeks.
Nose Enlargement (Augmented Rhinoplasty)
Augmented Rhinoplasty is a cosmetic procedure designed to improve the appearance of a flattened nose. It is carried out, under local anesthetic, by inserting a customized fabricated silicone framework inside the nose above the nasal bone. No hospitalization is required, with the operation taking approximately one hour, and patients can return to their residence immediately. Some swelling and bruising around the nose and beneath the eyes is unavoidable but this will disappear within 1-2 weeks.
This is not a procedure suitable for all, and Doctor Sanguan is very selective, but will usually be able to decide on the submission of a photograph.
Eyelid Surgery (Blepharoplasty)
Bletharoplasty is both a functional and cosmetic procedure aimed at reshaping the upper and/or lower eyelid. It involves the removal of excess tissue and the tightening of the surrounding muscles and skin.
Upper and lower eyelid surgery can both be carried out under local anesthetic, with each operation taking one and a half hours. Either or both can be carried out at the PPS clinic, and patients will need to return to the clinic for the removal of stitches 3-4 days later. Patients are advised to stay in Phuket for a minimum of 7 days. Some swelling will occur though this will fade in 7-10 days. During this time the wearing of sunglasses is recommended
Ear Surgery (Octoplasty)
Octoplasty involves changing the appearance of a person’s external ears, usually reducing their prominence by bringing them closer to the head. This is done by placing an incision at the back of the pinna (auricle) and cutting away the ear cartilage, allowing the ear to be repositioned closer to the head. The incision is then closed with nylon sutures.
Doctor Sanguan can perform this operation at the PPS clinic, with no hospitalization needed. The procedure takes approximately one hour, and the sutures will be removed 5-6 days after surgery.
Some bruising and swelling will occur, but will fade after1-2 weeks.
